A rare case of laryngeal dystonia associated with neurosyphilis: Response to botulinum toxin injection.Ho KH, Wright CC, Underbrink MP Department of Otolaryngology, University of Texas Medical Branch, Galveston, Texas, USA. kevinho.md@gmail.com We report a unique case of laryngeal dystonia in a 43-year-old male with neurosyphilis who underwent successful treatment with botulinum toxin injection. To date there have been no reports of laryngeal dystonia associated with neurosyphilis. The patient initially presented with strained and stuttering voice despite systemic penicillin therapy. After 2 months of speech therapy with limited relief, the patient received botulinum toxin injection to each thyroarytenoid muscle. Postinjection videostroboscopy showed marked improvement of voice quality. Published 24 December 2010 in Laryngoscope, 121(1): 147-9. Articles on Syphilis published 4 October 2010: Enhanced molecular typing of treponema pallidum: geographical distribution of strain types and association with neurosyphilis. J Infect Dis, 202(9): 1380-8. BACKGROUND: Strain typing is a tool for determining the diversity and epidemiology of infections. METHODS: Treponema pallidum DNA was isolated from 158 patients with syphilis from the United States, China, Ireland, and Madagascar and from 15 T. pallidum isolates. Six typing targets were assessed: (1) the number of 60‐bp repeats in the acidic repeat protein gene, (2) restriction fragment length polymorphism (RFLP) analysis of T. pallidum repeat (tpr) subfamily II genes, (3) RFLP analysis of ... Articles on Syphilis published 30 September 2010: Case report: Neuropathic arthropathy of the hip as a sequela of undiagnosed tertiary syphilis. Clin Orthop Relat Res, 468(11): 3126-31. BACKGROUND: Neuropathic arthropathy is characterized by rapidly progressive bone destruction in the setting of impaired nociceptive and proprioceptive innervation to the involved joint. It is seen most commonly in the foot and ankle, secondary to peripheral neuropathy in patients with diabetes mellitus. Other less common sites of involvement may include the knee, hip, shoulder, and spine, depending on the underlying etiology. Neuropathic arthropathy can be associated with tabes dorsalis, a ... Articles on Syphilis published 3 August 2010: Myositis as an unusual presentation of secondary syphilis. South Med J, 103(8): 807-8. Myositis is a complication of many infectious and noninfectious processes. Secondary syphilis is an uncommon cause of myositis, and, therefore, the diagnosis may be delayed. We describe a man with human immunodeficiency virus infection presenting with diffuse muscle injury secondary to syphilis. He had complete resolution of all signs and symptoms after treatment with intravenous penicillin. BACKGROUND: The Jarisch-Herxheimer reaction is an acute systemic event that can occur during the treatment of spirochetal infections. During pregnancy, it can cause signs and symptoms in both the mother and fetus, including fever, tachycardia, uterine contractions, and fetal heart rate pattern changes. CASE: A pregnant woman with limited prenatal care presented at 34 weeks of gestation in preterm labor with possible genital herpes. She received ampicillin for group B Streptococcus prophylaxis. ... [Abstract] [Full-text] Articles on Syphilis published 31 May 2010: Accelerating worldwide syphilis screening through rapid testing: a systematic review. Lancet Infect Dis, 10(6): 381-6. Syphilis is a persistent public health issue in many low-income countries that have limited capacity for testing, which traditionally relies on a sensitive non-treponemal test and then a specific treponemal test. However, the development of a new rapid treponemal test provides an opportunity to scale up syphilis screening in many settings where traditional tests are unavailable. This systematic review of immunochromatographic strip (ICS) syphilis tests describes the sensitivity and specificity ... J Infect Dis, 201(11): 1729-35. BACKGROUND: Syphilis remains an important source of morbidity worldwide. Long-acting penicillin is the only therapy currently recommended for syphilis in much of the world. Because of hesitation to use penicillin for fear of anaphylaxis, there is a need for an effective, well-tolerated alternative to penicillin for syphilis therapy. METHODS: This multicenter, randomized clinical trial was conducted in clinics for the treatment of persons with sexually transmitted diseases. We compared ...
